How to use NCLEX rationales so a wrong answer teaches you
The point of a practice question is the explanation after it. Read the stem again, write one sentence about the miss, and stop while the reason is clear.

A question you get right and never look at again did not teach you much. A question you get wrong, and then understand, changes the next one.
01Read the stem again
Before the rationale, name what the question was really asking: who is least stable, which action comes first, which finding is unexpected. If you cannot say that in one line, the miss was in the stem, not in the options.
02Separate the facts from the habit
Write one sentence: what you believed, and what the rationale says instead. That sentence is the review. Collecting fifty more items on the same day usually repeats the same miss.
03Stop while the reason is clear
Ten questions you can explain beat fifty you only scored. The score is a signal. The rationale is the lesson.
